Our Core Objective
To establish a single, auditable process for receiving, classifying, and managing all incident reports. This unified approach ensures safety, accountability, and continuous improvement across all operations.
A Universal Reporting System
All Contractors
Report All Incidents
To a Single Designated Channel
shoba.goorudat@exxonmobil.com &/or mail.list (2BD)
Typical Incident Distribution
This data is illustrative. Most incidents are minor, but all must be reported to identify trends and prevent escalation.
The Primary Reporting Tool
The **Initial Incident Report Form** is the mandatory first step for all reporting.
Key information captured includes a detailed description, consequences, immediate actions, and a preliminary classification.
The Contractor Reporting Flow
Incident Occurs
Secure the area and provide immediate aid.
Submit Initial Report

EMGL Review

Behind the Scenes: Report Classification
The Contractor Engagement & Data Analyst uses a two-part filter to determine the required follow-up protocol.
Question 1

Question 2
Is severity AHL 2+ or PHL 3+?
Decision Point
Both YES
Full Management Protocol
Incident entered into "Impact" software. A formal investigation report, based on our minimum requirements, will be required for closure.
Either NO
Record-Keeping Protocol
The initial report is archived for data and trend analysis. No further reports are required from the contractor.
The Full Management Protocol
For the most significant incidents, we ensure a thorough follow-up to prevent recurrence.
Investigation Report Standard
Instead of a rigid form, we require the contractor's standard investigation report to include our **minimum content requirements**.
- Executive Summary
- Detailed Incident Description
- Causal Analysis Findings
- Corrective & Preventive Actions
- Key Lessons Learned
This flexible approach respects contractor processes while ensuring high-quality, actionable findings.
